Abnormal renal function (dialysis, transplant, Cr > 2.26 mg/dL)
Abnormal liver function (cirrhosis or bilirubin > 2× ULN with AST/ALT/ALP > 3× ULN)
Prior stroke
Prior major bleeding or predisposition to bleeding
Labile INR (unstable, high, or time-in-therapeutic-range < 60%)
Concurrent antiplatelet or NSAID use
